MISFIT Program

We are glad that you have decided to participate in our MISFIT Program. Please remember, before joining our MISFIT program, you must:

1. Be a current patient at the MUSC Weight Management Center (including patients who are doing a maintenance contract)
2. Have watched all the videos and reviewed both forms on the previous webpage.

Below is the consent form for participation in our MISFIT program. Please thoroughly read it, then if you agree to the terms and conditions stipulated, complete and submit the form that is immediately below the consent form.
