Getting Started: Information for Patients
The best way to get started is to Request a referral for physical or occupational therapy from your doctor. A physician referral is not required to participate in therapy but most insurance companies require a physician referral before they will cover these services. Regardless of who refers you, where you go for therapy is up to you. So, do your research and find a therapy practice that is convenient for you and staffed with therapists that are qualified to address your particular needs.
Physicians
A complete referral is required for third party reimbursement for physical and/or occupational therapy services. The referral must contain the following information:

blue dotted arrow Diagnosis
blue dotted arrow Reason for referral
blue dotted arrow Primary insurance with pre-authorization if required
blue dotted arrow Frequency and duration
blue dotted arrow Date
blue dotted arrow Physician signature
Doctor completing form
