Getting Started: Information for Patients The best way to get started is toRequest a referral for physical or occupationaltherapy from your doctor. A physician referral is not required to participate intherapy but most insurance companies require a physician referral before they will cover these services. Regardless of who refers you, where you go for therapy is up to you. So, do your research and find atherapy practice that is convenient for you and staffed with therapists that are qualified to address your particular needs.
Physicians A complete referral is required for third party reimbursement for physical and/or occupational therapy services. The referral must contain the following information:
Patient's name Phone number Diagnosis Reason for referral Primary insurance with pre-authorization if required Frequency and duration Date Physician signature
