It is best described as a model or even a philosophy of how we can better take care of our patients. It is patient-centered; your needs and interests are of the highest importance to your primary care physician and your care team. Your care team is a group of people led by your Primary Care physician focused on the “whole you” and accountable for helping you navigate through the complex health system to connect all the episodes of care you receive.
A PCMH is a regular doctor’s office; however, it has met special criteria to be recognized as a PCMH. The goal is to provide a true partnership between patients and their health care team – including not only care, but value.
If you have high blood pressure, heart failure or diabetes, consider joining a PCMH. Patients with chronic conditions can have complex medical needs, ranging from basic preventive care to acute or end-of-life care.
With a medical “home,” you can count on a single, trusted doctor and health care team to provide comprehensive, coordinated care. Your PCMH team can help you manage your health, including getting preventative and follow-up care when needed.
