Physician Referral Information:
A Physician Referral form is required before an outpatient appointment can be scheduled. Referring physicians should call MEDULINE at 843-792-2200 or toll free at 800-922-5250.  MEDULINE provides you with direct access to the physicians and services at MUSC Medical Center.
