Movement Disorders : Making an Appointment : Information for Referring Physicians

To refer a patient to a movement disorders specialist, please call the scheduling office at (843) 792-3223 or fax the following information to us at (843) 792-1751:

  • Patient's full name
  • Date of birth
  • Social security number
  • Insurance
  • Diagnosis
  • Pertinent notes*

*If applicable, indicate if the patient should be added to our wait list in case an earlier appointment becomes available.

Scheduling will respond to the referring physician and mail the patient an information packet. Scheduling will only grant appointments using this procedure. Patients should not request appointments directly.

Making a Referral for Movement Disorder Surgery or Clinical Trials
Patients considering surgery for Parkinson's, tremor or other movement disorder must have a consultation prior to surgery. If you consider the patient a good candidate for surgery, you may wish to complete and fax the Expedited Consultation Form to (843) 792-1751. Patients eligible for clinical research studies or movement disorders surgery are often seen earlier in visits reserved in a separate schedule.  Expedited referral forms for new onset PD patients and clinical trials may be found below for your convenience.

Working with you
We are happy to help you care for your patient. Many patients need only one visit to clarify their situation or answer their questions. Many of our patients also live a considerable distance from the clinic. For these reasons, we are happy to provide their local physicians with guidance after the patient visit. Referring physicians usually receive consultation reports in about three or four weeks.
