Movement Disorders : Making an Appointment : Information for Patients

Getting a Referral
You must have a referral from your treating physician to see a movement disorders specialist.  Most insurers require this.

Make an Appointment
To make an appointment with a movement disorders specialist, the patient's physician must call (843) 792-3223 or fax the following information to us at (843) 792-1751:

  • Patient's full name
  • Date of birth
  • Social security number
  • Insurance
  • Diagnosis
  • Pertinent notes

NOTE: If applicable, the doctor should indicate if the patient should be added to our wait list in case an earlier appointment becomes available.

The scheduling office will contact you regarding the appointment and mail the patient an information packet. Scheduling will grant appointments only using this procedure. Patients should not request appointments directly.

Surgery Appointments
Patients considering surgery for Parkinson's, tremor or other movement disorder must have a movement disorders consultation prior to surgery. If the patient's doctor considers the patient a good candidate for surgery, have him or her contact us to schedule an appointment. If appropriate, the doctor may wish to complete and fax the Expedited Consultation Form.

Where to Go
Our consultations are performed in our outpatient clinic in Rutledge Tower on the main MUSC campus. Parking is available in the parking deck. Entrances to the parking deck are available from Rutledge and Ashley Avenue.

Research Patients
Research patients are seen at the Murray Center for Research on Parkinson's Disease and Related Disorders in the McClennan Banks building at the Charleston Memorial Hospital. If you participate in a trial, we will provide you with additional information at that time.

What to Bring
In order to get the most out of your visit, bring the following to your initial appointment:

  • All neurological records
  • If you have had a scan of your brain or spine, pick up the actual films (or images on disc) from your doctor's office or imaging center and bring to the appointment
  • A list of all medicines, including dosage. It's fine to bring pill bottles
  • Insurance cards
  • We ask you to fill out our information form prior to your visit and bring it with you. You may download it here.

 Patient Questionnaire

Each appointment takes about 1 hour, and patients generally are seen at their appointed times. If you are more than 30 minutes late for your appointment, you will be rescheduled. Please arrive early to allow time to park and register at the clinic. We suggest arriving at the parking area at least 45 minutes ahead of time.

Your Visit and Privacy
Because of Federal privacy regulations, we need written consent to communicate with anyone who is not present with you at your visit. We shall not routinely discuss your case by telephone with any other family member.  Records are released only after your written request. Your Movement Disorders physician cannot obtain records from another doctor's office for you. Your referring and treating physicians will receive copies of your consultation.

About our Wait Time
We are one of a few comprehensive movement disorders programs in the U.S., and the only program in South Carolina. We are committed to providing the best, most thorough care to our patients. Because of great demand for our services, patients may encounter a lengthy wait. Please understand that we must fulfill obligations to all patients. Patients will not be seen until the appointed time, and we cannot offer advice to patients before their visit. At the doctor's request, patients can be placed on a waiting list should an earlier appointment become available.

Patients eligible for clinical research studies or movement disorders surgery are often seen earlier in visits reserved in a separate schedule.

We are happy to work with your local health care providers. Many patients need only one visit to clarify their situation or answer their questions. Many of our patients also live a considerable distance from the clinic. For these reasons, we are happy to provide your local physicians with guidance following your visit.
